By signing up as a member with My Family Doctor, for a $300 annual membership fee ($100 for infants in the first year of life)*, you are entitled to the following services:

• A 1-hour comprehensive annual physical exam (valued at $300) with age and sex-appropriate screening tests (e.g. pap smear, breast exam, prostate exam, etc.) to address your health goals and challenges. If a complete physical is not needed or desired, this time can also be used towards other medical care or homeopathic consultation.  Annual exam must be scheduled by the patient within one year of joining.
• A discounted rate of 30% on office visits
• 24 hour on-call phone consultation with a physician for urgent matters
• Same day appointment availability for urgent matters when scheduled by 12 noon
  
• Communicate with your physician via secure email program (www.reachmydoctor.com)
• Manage your personal health record on-line
• Receive lab and imaging results via e-mail with your physician’s comments
• Request prescription refills on-line

* Payment plans are available.  Please contact the office via email if you would like to set this up.  Patients with Medicaid  do not pay an annual fee.
