RATE CHANGE- Effective April 1, 2012
- Annual membership $350 (includes one hour annual exam and 30% discount on visit rates)
- Member Hourly Rate for acute/follow up visits: $220/hour ($110/30 minutes; $75/20 minutes)
- Non-Member Hourly Rate $330/hour ($165/30 minute)
Additional Fees May Apply:
$15 for a rapid strep test
$25 for a suture kit
$10 for a urinalysis or urine pregnancy test
ON-SITE LOW COST PHARMACEUTICALS ($15) Penicillin; Azithromycin (Z-Pack); Ondansetron (Zofran, for nausea and vomiting); Doxycyline; A/B Otic (ear drops for ear pain/infection); Acyclovir (antiviral for herpes); Bactrim; Ciprofloxacin; Flagyl; and Afrin nasal spray.
FOR PATIENTS WITH INSURANCE, ALL LABS AND IMAGING STUDIES ARE BILLED THROUGH INSURANCE (i.e. patients do NOT pay out of pocket for these tests).
